A Form: Completion & Submission

At the time of submission of the A Form, each user declared as participating in an experimental session must have a valid SAFETY TRAINING COURSE and must have validated his/her personal "USER & SAFETY DECLARATION FORM" relevant to that particular experimental session. The deadline for submission of the A Form is at least 8 DAYS before the experiment is due to start.
